  • Is there an application form to complete in order to appoint a legal custodian?

    Yes. To appoint a custodian, A form called Custodianship Declaration form (IMM5646E) has to be completed and notarized.

    There are two pages of the form. The two pages of the form look similar, but:

    • Page 1 (CUSTODIAN FOR MINORS STUDYING IN CANADA) needs to be signed by the custodian and notarized with a lawyer or at a notary public in Canada
    • Page 2 (PARENTS/GUARDIANS FOR MINORS STUDYING IN CANADA) needs to be signed by the parents or legal guardians of the minor child and notarized in their home country

  • When to complete the form?

    Only when you are assigned as a custodian for a student on Homadorma, we will send the form to you with that student’s information filled in for you to notarize.

  • How to notarize the IMM5646E form?

    1. When a student books your Custodianship Service, the student information and your information will be filled out on the IMM5646E form by Homadorma
    2. You will receive a filled form, page 1 - CUSTODIAN FOR MINORS STUDYING IN CANADA by email from homestay@homadorma.com
    3. Once you receive the form, you have 5 days to complete the notarization with your local lawyer or notary public. The custodian is responsible for the cost of notarization.
    4. Send a scanned copy of your notarized form to our support team at homestay@homadorma.com for approval
    5. Once your form is approved by our staff, please mail out the form to your student within 1 business day by Canada Post registered mail and send the tracking number to our support team at homestay@homadorma.com.
    6. You are responsible for submitting the receipt from Canada Post to homestay@homadorma.com. Homadorma will reimburse the mailing cost.
    Your students also have the responsibility to complete the following application procedure:
    1. Student’s parent(s) will need to complete and notarize the page 2 - PARENTS/GUARDIANS FOR MINORS STUDYING IN CANADA in the country of origin
    2. Student should bring the complete IMM5646E form with him/her when entering Canada and the entire school registration process
