  • Is there an application form to complete in order to appoint a legal custodian?

    Yes. To appoint a custodian, A form called Custodianship Declaration form (IMM5646E) has to be completed and notarized.

    There are two pages of the form. The two pages of the form look similar, but:

    • Page 1 (CUSTODIAN FOR MINORS STUDYING IN CANADA) needs to be signed by the custodian and notarized with a lawyer or at a notary public in Canada
    • Page 2 (PARENTS/GUARDIANS FOR MINORS STUDYING IN CANADA) needs to be signed by the parents or legal guardians of the minor child and notarized in their home country

  • How to complete the form?

    Homadorma’s part

    After you book a custodian on Homadorma, Homadorma will put your information on the form and send the form to your custodian for him/her to notarize. Once the notarization process completes, your custodian will mail the form to you by Canada Post within one business day.

    • Your parent(s) will need to complete and notarize the page 2 - PARENTS/GUARDIANS FOR MINORS STUDYING IN CANADA in the country of origin
    • You should bring the complete IMM5646E form with you when entering Canada and for the entire school registration process
